This form is intended to connect agricultural producers and outfitters affected by the Lee and Elk Fires with essential resources to address both immediate needs and long-term recovery and mitigation efforts. We recognize the sensitivity of this information—specific producer data will not be shared with the public. This questionnaire was developed by the White River and Douglas Creek Conservation Districts and Rio Blanco County Natural Resources Department.
